  1. Article 1: Pressure Injury Prevention in a Geriatric Post-Acute Care Setting: A Quality Improvement Study

What are the advantages and disadvantages of implementing the article recommendations on your unit and/or hospital?
According to Hartung et al, three ideas in a period of six-months were implemented to prevent stage II pressure injuries (PI). The QI project quality included 1) point of care nursing-led PI prevalence and incidence study; 2) standardized positioning plans from an inter-disciplinary bedside assessment used, and 3) revising a current mattress algorithm.
The advantages of this study is that this QI study included a diverse team such a two advance practice nursing team, personal support member and an external mattress company employee. The advantage is that experienced and possibly highly trained core leaders have more knowledge, influence to convert effective practices into policy. The use of pictures to aid nursing staff was effective educational material.
Also, inviting a mattress company’s member which elevates the vision of developing products to prevent and treat current PIs is essential to a nursing bundle to treat/prevent stage II PIs. The disadvantage is that the at least one bedside nurse should have been involved to enact the QI for a longer period of time. Involving bedside staff would make the nursing team part of a project that would increase adherence and compliance with the project.

Describe the method used by the author of the study
A quantitative quality improvement project included several outlined methods from Health Quality Ontario from RNAOs Implementation of Best Practices Guideline Toolkit. Possible implement challenge kibosh (PICK) charts, inter-professional working groups, PDSA cycles performance boards and pertinent auditing. This is evidenced practice I widely used and effective in healthcare projects.

  2. What are the advantages and disadvantages of implementing the article recommendations on your unit and/or hospital?

This info-gram discusses the suggested implementations, based on the National Pressure Injury Advisory Panel (NPIAP), to help and reduce facial injuries due to personal protective equipment (PPE). The advantages of the proposed recommendations in this info-gram include increased caregiver awareness, protection, and safety in regards to preventing pressure-related skin injuries caused by the use of PPE in the clinical setting. During this COVID pandemic, caregivers are required to wear proper PPE when caring for both COVID-positive and COVID-negative patients to protect themselves from potential transmission of the virus. All caregivers are required to wear a surgical mask for their entire shift, causing the potential for skin integrity breakdown related to continuous PPE use. This info-gram is a great tool to use within the clinical setting as it allows caregivers the option to protect themselvers appropriately during PPE use. A disadvantage from this info-gram may be situations where skin reactions occur secondary to the use of "SurePrep" and "Optifoam Thin". Some individuals may have an allergic reaction to the chemicals/materials used within these products; and therefore, would require alternative preventative options.

Discuss the limitations of the article: limited sample size, design flaws, and/or author bias.

The limitations of the article includes the lack of indicated studies regarding the efficacy of the preventative measure products, as well as the lack of alternative measures (for caregivers with sensitive skin) in preventing pressure related injuries with PPE use. It may also be helpful to demonstrate, via illustrations, how the products are properly applied and used.
