    Discuss the sample size used in the study.
    There were 16,762 potential participants from databases in the U.S., Canada, and the U.K. From 2012 to 2013, 3,146 people responded to the email and became participants. All of these participants had gone through ostomy surgery. 52.9% were male and 47.1% were female. Colostomy patients represented the majority of the participants.
    Describe the method used. Was the correct method used.
    The authors of this study used random sampling from three data bases in Canada, the U.K. and the U.S. They were emailed and asked to do a self assessment of their peristomal skin, a Qol (quality of life)self rating scale, and a QALD (quality of life days)scale. I think the method used by the authors was correct. They were able to analyze the relationship between poor peristomal health and quality of life. It was found there is a linear relationship between improved peristomal skin health and Qol.

    - The author used a cross-sectional survey that provides an eight domain profile of the health and wellbeing of an adult (aged 18 years and older) target population, and includes psychometric summary measures of physical and mental health.

    Discuss the limitation of the article: limited sample size, design flaws, and/or author bias.
    - The limitation of the article is that it is unable to determine the cause and effect between a successful skin intervention and quality of life, health utility, and quality adjusted life days.

    Data collection was done via random sampling from a database from US, Canada, and UK of patients who have done through ostomy surgery. The n=16762 patients within a 24 month period. However, a survey was sent out and only 3146 responded.
    Discuss the limitations of the article: limited sample size, design flaws, and/or author bias
    Data was evaluated and analyzed using SAS and Cronbach's alpha which shows to be reliable, however, the data is extracted via surveys which leaves room for bias and error, it required self assessment of the stoma as well.

  4. 1. What are the advantages of implementing the article recommendations on your unit and or hospital?

    One of the treatments for gastrointestinal diseases and colon cancer is surgery and placement of ostomy. Unfortunately irritated peristomal skin remains as one of the major complications. This study was aimed to assess the impact of damage peristomal skin on the health utility and quality adjusted life days in an adult ostomy population. This study was done via survey form which was sent to 3146 respondents. The statistical significant results were found in tables 4, 5 and 6 which demonstrate a linear decrease in QOL, health utility and QALD respectively associated with worsening peristomal skin conditions. Patient admitted in the hospital with irritated peristomal skin should not only be treated for dermal conditions. As shown in this study, this can affect or influence the quality of life. Patient should be educated on ostomy care, proper use of products, and recognition of skin conditions and seek proper help. In the hospital, wound care nurses are consulted to see patients with complicated or severe peristomal skin irritations. A successful treatment intervention in peristomal skin improves quality of life.

    There are 16,762 potential participants from USA Canada in UK who have undergone ostomy surgery. 3146 responded and completed the survey, giving a response rate of 18.8%.

  5. Describe the method used by the author of the study
    A generic health-related QoL survey, was chosen to assess population of adults residing in the USA, UK, and Canada.. This is a short form health survey that provides an eight domain profile of the health and wellbeing of an adult (aged 18 years and older) t, and includes psychometric summary measures of physical and mental health.
    The survey included a visual analogue scale of QoL presented as a self-rating scale (scale 0 to 100, where 100 is best possible), and normed to a mean of 50 and a standard deviation of 10.Mean scores in the range of 45 to 55 are considered representative of the average of the sample. Scores above 55 represent an above-average QoL, scores of between 40 and 45 represent marginal impairments in QoL, and scores below 40 indicate significant impairments or limitations.
    Respondents provided their perception of skin integrity such as Normal skin, mild skin irritation, moderate skin irritation, and severe skin irritation.

    Was the correct method used? Why or why not?
    I believe the method is correct. Pt will always have impaired body image once they have a stoma. Very rare that a person will look at their stool after having a BM. Plus the odor of the stool and depends on what they eat and consistency of the stool. The Quality of Life is disrupted and having an irritated skin and dealing with discomfort from skin irritation will just make it worst, no matter what part of the world, what kind of culture or religion we came from.

    2. Limitations of study

    Study is based on the perception of the participants regarding skin irritation levels. Although the association found between peristomal skin health and quality of life, health utility and quality adjusted life days are strongly suggestive of overall benefit that a successful skin care intervention can have, the cause and effect cannot be determined through this study.
