Submission Instructions

Please follow the steps in order to get your certificate (affidavit)  for the Wound/Ostomy Journal:
1) complete the required number of articles with corresponding responses. 

• 4 Articles and 2 questions each.
• Responses for each question requires a detailed discussion and expected to be explained with at least 3 sentences. Review of responses may require resubmission of answer.

2) email either:

• Lupie Bautista (lupie.bautista@providence.org)
• Melissa Godfrey (melissa.godfrey@providence.org).

3) Include in your email the following details:

•   - Your full name (in case you are using a nickname on your blogger ID).
•   - The months or titles of articles you completed

4) Please allow at least  1 week to receive your  certificate. There is no guarantee that a certificate will be given to any completed works sent to the department less than 1 week before the deadline of  the current year's clinical ladder (September 30).
